Tired on a GLP-1? Food and sleep factors behind Ozempic fatigue

Yes, feeling tired is a known side effect of GLP-1 medicines. In the Wegovy weight-loss trials, 11% of people on the medicine reported fatigue versus 5% on placebo, and the Zepbound trials reported 5% to 7% versus 3%. Often the bigger drivers are things you can work on: eating much less than before, too little protein, not drinking enough when nausea or diarrhea hits, and short sleep. Low blood sugar is a risk if you also take insulin or a sulfonylurea. Call your doctor if fatigue is severe, keeps getting worse, or comes with dizziness, confusion or very little urine.

What the labels say about fatigue

The FDA-approved prescribing information is the best place to start, because it reports what happened in controlled trials.

  • Wegovy: In the trials for weight reduction, fatigue (which the label counts together with weakness, or asthenia) was reported by 11% of adults on Wegovy injection versus 5% on placebo. The patient information lists "tiredness (fatigue)" among the most common side effects.
  • Zepbound: In its weight-reduction trials, fatigue (counted together with weakness, lethargy and malaise) was reported by 5% to 7% of people across the doses studied, versus 3% on placebo.
  • Ozempic: The label lists fatigue among less common reactions in its diabetes trials.

So tiredness is more common on these medicines than on placebo, but most people in the trials didn't report it. The Wegovy label says its stomach side effects were reported most often while the dose was being increased. Don't change your dose on your own; talk with your prescriber.

Why GLP-1s can leave you tired

There usually isn't one cause. These are the most common contributors.

You're eating a lot less

That's the point of the medicine, but it has a cost. A 2025 joint advisory from four obesity and nutrition groups, including the American Society for Nutrition and The Obesity Society, says people on GLP-1s for obesity cut how much they eat by about 16% to 39%. It warns that a large, rapid drop like that can mean too few essential vitamins and minerals, and it lists "fatigue beyond expected levels" among signs of a true nutrient deficiency. Our guide to GLP-1s and nutrient deficiencies covers which ones to ask about.

Too little protein

The same advisory notes that eating less protein because of a smaller appetite can add to muscle loss. Losing muscle can make everyday activity feel harder. See our guide to getting enough protein on a GLP-1.

Not enough fluid

Nausea, vomiting and diarrhea make it easy to fall behind on fluids, and a smaller appetite can mean you drink less too. MedlinePlus lists headache, muscle cramps, thirst and darker urine among signs of mild to moderate dehydration, and dizziness, confusion and listlessness among signs of severe dehydration. The Wegovy label also warns that dehydration from stomach side effects has been linked to kidney problems.

Fatigue is easier to sort out when you can see your patterns. Olive is a health companion that checks in with you every day, looks at what you eat, and helps you build small habits that actually hold. It doesn't diagnose or treat anything, so questions about your medicine still belong with your prescriber.

Low blood sugar, if you take other diabetes medicines

The Wegovy label warns that using it with insulin or an insulin secretagogue, such as a sulfonylurea, raises the risk of low blood sugar. NIDDK lists feeling tired, shaky, hungry, dizzy or irritable among mild to moderate symptoms. If you take these medicines and feel this way, check your blood sugar and follow the plan your care team gave you.

Short sleep

NHLBI recommends 7 to 9 hours of sleep a night for adults and says sleep deficiency can make it hard to focus and react. If nausea, heartburn or bathroom trips are waking you up, that adds up fast.

Something else is going on

Fatigue has many causes that have nothing to do with your medicine. MedlinePlus lists anemia and iron deficiency, depression, sleep disorders like sleep apnea, an underactive or overactive thyroid, and some medicines, including antihistamines, blood pressure medicines and sleeping pills. NHLBI notes that iron-deficiency anemia can cause tiredness, dizziness and cold hands and feet. A simple blood test can check. Before your visit, note how long the tiredness has lasted, whether it's worse at certain times of day, what you've been eating and drinking, and any other medicines you started around the same time.

Food habits that help

  • Eat something regular. Small meals spread through the day are easier than skipping meals and running on empty. If nausea makes that hard, our hub on what to eat when a GLP-1 makes you nauseous has foods that tend to sit well.
  • Lead with protein. Put eggs, Greek yogurt, fish, chicken, beans or tofu first, so it gets eaten before you fill up.
  • Make every bite count. With less room, choose vegetables, fruit, whole grains and protein over snacks with little nutrition.
  • Sip all day. Keep water nearby and drink small amounts often, especially on days with vomiting or diarrhea. Our guide to what to eat when a GLP-1 causes diarrhea covers fluids too.
  • Don't lean on caffeine. MedlinePlus says stimulants, including caffeine, aren't effective treatments for fatigue and can make it worse when you stop.
  • Ask before adding supplements. MedlinePlus suggests discussing with your provider whether any vitamins might help, rather than guessing.

Sleep habits that help

  • Aim for 7 to 9 hours. Keep the same bedtime and wake time, even on weekends.
  • Eat dinner earlier and lighter. A full stomach at bedtime can bring on reflux and nausea, which disrupt sleep.
  • Limit alcohol. MedlinePlus suggests avoiding alcohol when you're fatigued, and it can make low blood sugar harder to notice.
  • Move a little each day. MedlinePlus lists regular exercise among ways to reduce fatigue. A short walk after a meal counts.

When to call your doctor

MedlinePlus says to contact your provider right away if fatigue comes with:

  • Confusion or dizziness
  • Blurred vision
  • Little or no urine
  • Recent swelling and weight gain
  • Thoughts of harming yourself

Make an appointment if you have unexplained weakness or fatigue, especially with fever, weight loss you can't explain or regular sweats, or if you also have constipation, dry skin or trouble tolerating cold. Tell your prescriber if you can't keep fluids down. Severe low blood sugar is dangerous and needs treatment right away.

This guide is general information, not medical advice. Talk with your prescriber about side effects, and don't start, stop or change any medicine or supplement without them.

Steadier energy, one day at a time

Small changes to how you eat, drink and sleep usually make the biggest difference. Olive checks in with you every day and notices what's keeping your body stuck.

If steady habits haven't moved stubborn weight, Olive's Protocol is one option to look into. Prescriptions are only written after a medical consultation with a licensed provider. Not everyone qualifies.

Questions

Does Ozempic make you tired?

It can. Fatigue is a listed side effect of semaglutide medicines like Ozempic and Wegovy, though most people in the trials didn't report it.

How common is fatigue on Wegovy or Zepbound?

In the weight-loss trials, 11% of people on Wegovy reported fatigue versus 5% on placebo. On Zepbound it was 5% to 7%, versus 3% on placebo.

Why do I feel so tired on a GLP-1?

Common reasons include eating much less, too little protein, not enough fluid, short sleep and, with some diabetes medicines, low blood sugar.

Does fatigue on a GLP-1 go away?

Stomach side effects, which can leave you short on food and fluids, were most common while the Wegovy dose was being increased. If it lasts or gets worse, talk with your prescriber and ask about checking for other causes.

What should I eat for energy on a GLP-1?

Regular small meals that lead with protein, plus vegetables, fruit and whole grains. Sip fluids through the day, especially if you have vomiting or diarrhea.

When is tiredness on a GLP-1 serious?

Call right away for confusion, dizziness, blurred vision or very little urine. Severe low blood sugar needs urgent treatment.
